Reporting to us after your event is mandatory. These reports are essential for our Ministry funders and insurance providers. The forms provided below assist both parties in gathering the required information. These forms are mandatory for all events.
Incident Reporting Protocol
In the event of any incident occurring during or in relation to your event, particularly those requiring medical intervention, it is imperative to complete and submit an incident report using the provided form. This form aligns with the expectations of our insurance provider.
Organizers are required to submit their completed Sanctioning Request Form and EAP for their event early each year. By completing the form, organizers agree to become members of OMCKRA, adhere to the safety policy (including obtaining waivers from all participants), and submit fees, counts, and participant information for Ministry reporting and insurance purposes within a few weeks of the event. The Sanctioning Committee reviews the application for sanctioning and will communicate any questions or concerns before confirming event approval.
